Adams County Health Center (ACHC) is a Federally Qualified Health Center that strives to provide high quality health care services to residents and visitors of Adams, Washington, Valley, Idaho and surrounding counties. ACHC is a multidisciplinary clinic where medical, behavioral health, dental, optometry, physical therapy, care management, laboratory, and pharmacy professionals all work together to give the best care possible to our patients!

With healthcare facilities few and far between, ACHC offers an array of services to meet the healthcare needs of everyone, regardless of social or economic status. Some of the services ACHC currently offers are primary care, dental, pharmacy, laboratory, optometry, behavioral health, physical therapy, diabetes care, immunizations, several free community outreach programs, and financial and insurance programs.

Dr. Jordan Blanchard

Medical Director

After Jordan Blanchard graduated from Brigham Young University with a Bachelor of Science in Biology, he attended the University of Utah School of Medicine, where he received his Medical Degree in 2011.

Dr. Blanchard completed his residency at the Utah Valley Family Medicine Residency in 2014 and is excited to live and work in Weiser and Council. Dr. Blanchard enjoys getting to know his patients and be able to follow them through a lifetime of care. His interests include women's and men's health, pediatrics, seniors and urgent care. Dr. Blanchard is now offering DOT physicals at ACHC.

Chelsy Taylor

Physician Assistant

Chelsy Taylor is a Physician Assistant who received her undergraduate degree in health promotion from the University of Idaho and her Master of Physician Assistant Studies from Idaho State University in 2013. Chelsy enjoys providing care for patients of all ages, from wellness visits to management of acute and chronic conditions. She is a native of New Meadows and is happy to have returned home in 2022, after living and practicing medicine in Northern Idaho and Eastern Washington since graduating from PA school. Outside of work she enjoys time with her husband and two young children, helping on her family’s ranch, skiing, hiking, and being outdoors.

Amy Beam

Family Nurse Practitioner

Amy Beam is one of our own Idaho natives who has a passion for rural life and loves her community. She and her husband live just North of Council. She has 2 sons and 3 grandchildren, whom she adores. Amy likes to spend her free time with her grandkids, pets, and gardening. Amy is a bright, funny and caring person, and this sentence made her laugh! Her life motto is “One person can make a difference.†Amy received her BS in Nursing from Idaho State University, her MSN Family Nurse Practitioner degree from Northwest Nazarene University, and her Post-Masters Certificate in Psychiatric Mental Health from Norwich University. All that schooling resulted in what Amy lovingly describes as alphabet soup degrees (RN, BSN, FNP, PMHNP) listed after her name.

Before she knew ACHC existed, Amy knew she wanted to serve the Council community and surrounding rural area. Amy spent her clinical hours during nurse practitioner training here at ACHC and knew it was the right place for her. With over 30 years’ healthcare experience, she loves serving all ages, throughout the lifespan.  She cherishes the relationships and sense of community she has built with her patients and their families, noting that there is always room for more. Amy will work patiently with each of you to promote wellness and holistic self-care whenever possible and will respect your choices should you opt out of guideline-directed care.

Julie Kimpel

Physician Assistant

Julie Kimpel is a Physician Assistant (PA) who graduated from the Idaho State University PA program in 2024 and started working at ACHC shortly thereafter. She completed a Doctorate of Medical Science (DMS) degree from Butler University and bachelor's and master's degrees in biology from University of Wisconsin-Parkside and Fort Hays State University, respectively. She was a NAIA/Division II cross-country and track athlete and ran several marathons, including the Boston Marathon. Prior to starting her PA education, she taught anatomy & physiology and other courses at BYU-Idaho and College of Eastern Idaho. Before that she spent many years as a stay-at-home mom and owned West View Horsemanship where she taught horseback riding lessons and trained horses. Outside of work, Julie enjoys spending time with her husband and four adult children, riding horses, hiking, and cycling.

Dr. Monte Eppich

Doctor of Dental Surgery & Dental Director

Dr. Eppich graduated with a Bachelor of Science in Biology from Villanova University in 1996 and from Creighton University School of Dentistry in 2001. After completing a post-doctorate in Advanced Education in General Dentistry from Idaho State University, Dr. Eppich relocated to Council. Splitting his time between his private practice and ACHC, Dr. Eppich also serves as a preceptor for ISU the Advanced Education in General Dentistry rural rotation, which takes place at ACHC. Dr. Eppich enjoys living in the Council community, he has spent time on numerous local boards and organizations. He spends his free time with his wife and four children exploring the outdoors, hunting, fishing, hiking, camping, skiing, coaching and being a dad.

Dr. Gregory Smith

Doctor of Dental Surgery

Dr. Smith graduated with a Bachelor of Science in Business Administration U.C. Berkeley in 1985, and a Juris Doctor from U.C. Los Angeles in 1989. After practicing law in Alabama, Dr. Smith graduated with a Doctor of Dental Surgery from U.C. San Francisco in 1998. He then took over his father’s dental practice in Norwalk, and practiced there over 20 years. Two separate cars crashed into his medical office building and shut down his practice, forcing Dr. Smith to relocate. He and his wife, who has family in Idaho and Montana, and his older sons all enjoy fishing, hunting and skiing.

Kaylee Zielinski

Registered Dental Hygienist

Kaylee is a Council, Idaho native who graduated with an Associate in Applied Science degree in Dental Hygiene from Remington College in Nashville, TN in 2014. Their family spent five years in Kentucky and three years in Washington while her husband was in the Army. Kaylee is very excited to be back in Council to serve the community she loves with her husband and three children.

Heather Bergstrom

LCSW (Licensed Clinical Social Worker) is available via telehealth Tues. & Wed. and is in the office on Thur. & Fri.

Heather received her master’s degree in social work from Portland State University in 1990. She is a Licensed Clinical Social Worker in Idaho and Oregon. She has lived in Ontario, Oregon since 1994, practicing in medical and mental health settings prior to coming to Adams County Health Center. She enjoys working with people of all ages to support their emotional wellness and personal growth using a variety of techniques, including cognitive behavioral therapy, mindfulness, solution-focused, and person-centered therapy modalities.  

Heather is certified in Lifestyle Medicine, Cognitive Behavioral Therapy for Insomnia, and is working toward certification in Accelerated Resolution Therapy. She has also completed a 200-hour yoga teacher training and has received advanced training in mind-body medicine through the Center for Mind-Body Medicine. She enjoys spending time with family and friends, quilting, knitting, yoga, and outdoor activities.

Sue Warner

Family Nurse Practitioner

Sue Warner is a Family Nurse Practitioner who has been with ACHC for over 35 years. She is a certified Diabetes Care and Education Specialist who specializes in Diabetes Self-Management Education. She received her Nursing degree from Deaconess Hospital School of Nursing in Spokane, WA. Sue then went on to graduate from Concordia University Wisconsin in Mequon, WI with a Master of Science in Nursing - Nurse Practitioner degree. Having been raised here, Sue is honored to provide quality health care to her friends and neighbors and any who visit here. Sue enjoys spending time with her family and participating in Eastern Star.

OPTOMETRY PROVIDERS

Dr. Kevin McSweeney

Optometrist

Dr. McSweeney grew up on the east coast and served 6 years in the Marine Corps with a tour in Vietnam. After graduating Illinois College of Optometry, he opened a private Optometry office in Maryland, which he operated for four years. He then joined the Katzen Eye Group, an Ophthalmology practice in Baltimore. Being an avid outdoorsman with a love for skiing, kayaking and mountain biking, he moved to Idaho to work with Dr. John Sonntag in his Boise Ophthalmology practice. Taking a sabbatical from Optometry, Dr. McSweeney spent a year mentoring troubled young boys at Wisdom Ranch, a “working cow/calf outfit†in Arco, Idaho. He most recently worked at the Hollingshead Eye Center, an Ophthalmology practice in Boise, prior to joining the Adams County Health Center to open an Optometry Clinic offering full eye care services for the community.

Carlene Emerson

Pharmacist

Carlene is a pharmacist licensed to practice pharmacy in both the states of Idaho and Washington with 16 years of pharmacy experience. She graduated from the University of Nebraska Kearney with a BS degree in Medical Technology. Carlene then worked as a Medical Technician in the laboratory of both hospital and clinic settings for 19 years, 13 of those were at the Washington State University (WSU) Veterinary Teaching Hospital. She then worked in research at WSU for several years before deciding to attend pharmacy school. Carlene graduated with a Doctor of Pharmacy degree in 2008 from WSU. Since graduation, she has worked as a pharmacist in both retail and clinical settings. She feels so fortunate to have found her ideal career niche at ACHC where she works alongside the providers to ensure the best care possible is given to our patients and every person in this community. Carlene and her husband have 2 children, 4 grandchildren and 1 great granddaughter. They enjoy small town living, like to travel, camping across our beautiful state and country, and the thing they enjoy most is spending time with their family.

Jean Cole

Physical Therapist

Jean received her undergraduate degree from The University of Colorado, Boulder, including a semester abroad and her Physical Therapy graduate degree from the University of Colorado Health Sciences Center in Denver, CO. Previously, Jean worked for St. Luke's in Ketchum where she treated inpatients and outpatients and performed aquatic therapy & treadmill stress tests. She also worked in home health for several years.

At ACHC, Jean treats post-surgical patients, sports related injuries, and neurological and musculoskeletal conditions including chronic pain, headaches, and muscle spasms.  She utilizes a variety of techniques such as joint mobilization, therapeutic exercise, taping, patient education and dry needling upon patient consent, which is similar to acupuncture.  The application of the acupuncture needles has been shown to improve mobility and strength while decreasing pain and inflammation. Thus, it’s effective in treating chronic pain, sports related injuries, headaches and muscle spasms. She enjoys recreational activities including skiing, hiking and kayaking.

Jackie Caba

Community Health Worker (CHW)

Jackie Caba is a Community Health Worker dedicated to promoting health equity and empowering individuals through education and advocacy. Her passion for helping others began at age 14, when she volunteered as a candy striper at the Boise VA Hospital—an experience that sparked a lifelong commitment to compassionate care and service.

Born in Council, Idaho, and a proud graduate of Mountain Home High School and North Idaho College, Jackie brings a deep understanding of the challenges and strengths found in rural communities. This connection fuels her advocacy for accessible, community-based healthcare solutions in underserved regions.

With a strong foundation in population health strategies and value-based care, Jackie supports communities in navigating healthcare systems and accessing essential services.

Outside of work, Jackie enjoys spending time with her wonderful husband and bird dogs, exploring the mountains in their UTV, baking the (self-proclaimed) best banana bread, reading, and game nights with family & friends.
