ACHC, in partnership with Idaho State University, Bengal Pharmacy, has a full-service pharmacy / telepharmacy.  In this new model, a pharmacist will either be on-site or will be available via live video link for consultations with patients and a Certified Pharmacy Technician will be in the Pharmacy to distribute prescriptions.

LABORATORY

ACHC is a CLIA certified laboratory, which means that we are certified by the Center for Medicare and Medicaid Services (CMS) to collect and process human samples for diagnostic testing.

Orders from outside (non ACHC) primary care providers:
 
ACHC Lab can draw blood and collect other specimens for outside providers.  Labs are sent out to Treasure valley Lab or Express Labs to be run and processed.  The lab sends the results vis fax to the ordering provider.  The provider or facility is then responsible for informing the patient of the results.
 
All outside orders must be received by ACHC lab staff members prior to scheduling appointments

Behavioral Health

ACHC's Behavioral Health Team includes a Psychiatric Nurse Practitioner and a Licensed Clinical Social Worker who work together with our health care providers to assist you with behavioral health needs including medication management and counseling. We use a variety of trauma-informed, evidence-based approaches to help you manage a broad range of behavioral health conditions, including but not limited to depression, anxiety, trauma, anger management, or substance use issues. We can also help you develop coping strategies to manage situational stressors related to relationships, work, and school. We use a person-centered approach, helping you to find the tools and skills to help you with whatever your problem or issue may be. SLIDING FEE DISCOUNT PROGRAM

ACHC offers a Sliding Fee Discount program!  All patients are eligible to apply, even those who have insurance.  Enrollment forms are available on the link here or in the clinic. Applications must be renewed annually and anytime there is a change in an enrollee's financial status. It is preferred to have completed applications prior to your appointment date.  Paperwork takes approximately 30 minutes to complete and you will need an Adult Drivers License or ID Card, Minor Identification (S.S. Good Faith Estimate

You have the right to receive a “Good Faith Estimate†explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.  

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. A Good Faith Estimate shows the cost of items and services that are reasonably expected based on your healthcare needs at the Adams County Health Center. This estimate is based on our understanding of your needs for that visit. 

While caring for you, our providers may recommend additional services not listed here, or unknown or unexpected costs may arise during your service. Because of this, your final charges may differ from the estimate set forth in this document. Additionally, you may be billed from other third parties for services such as lab tests or other diagnostics that are sent out of our clinic.

The Adams County Health Center offers sliding discounts based on your income and the number of people in your household.  ACHC staff will help you determine which payment group you belong to.  

Make sure you receive your Good Faith Estimate from the ACHC in writing at least 1 business day before your medical service or item. You can also ask ACHC staff, including your health care provider, or any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 

Make sure to save a copy or picture of your Good Faith Estimate. You may need it if you are billed a higher amount.

Free Type 1 Diabetes Risk Screening at ACHC

T1D FREE Screening Days at ACHC occur on the first Wednesday of every month! Just call 208-253-4242 to schedule an appointment! You don't have to have symptoms or a family history of T1D to be screened! This screening can be done on CHILDREN and ADULTS and is just a poke to the finger! It is available to anyone who would like to be screened for T1D, regardless of if you are a patient of ACHC. What is Type 1 Diabetes?

Did you know that over 90% of people diagnosed with type 1 diabetes don’t have a family connection? Type 1 diabetes is a chronic, autoimmune disease that can strike both children and adults. A simple screening test can help families determine, before symptoms even occur, if they may be at risk. Because most people do not have a family history of type 1 diabetes, symptoms and a diagnosis often come out of the blue. And because families don’t know what to look for, many people require hospitalization at diagnosis because their blood sugars are very high and the person is very sick. If not treated properly, people with T1D are vulnerable to health issues ranging from minor to severe. Most people with T1D spend the majority of their time with blood glucose levels outside the recommended healthy range, which can lead to potentially deadly episodes of hyperglycemia (high blood sugar) and hypoglycemia (low blood sugar). Chronic high blood sugar often causes devastating health complications later in life, including blindness, kidney failure, heart disease and nerve damage that can lead to amputations.

ACHC's Diabetes Prevention Program (DPP)

What is DPP?

DPP teaches that weight loss is the best way to decrease the risk of being diagnosed with type 2 diabetes. The program encourages members to work together as a team to support each other to reach weight loss goals and to stay motivated to take charge of their own health to prevent being diagnosed with type 2 diabetes. During the course, students will learn that weight loss can not only help them prevent becoming a type 2 diabetic, but that losing weight can also help lower blood pressure and cholesterol, which leads to an even healthier lifestyle.

ACHC’s Diabetes Self-Management Education and Support Program

Adams County Health Center’s (ACHC) Diabetes Self-Management Education and Support (DSMES) Program has achieved the significant milestone of accreditation by the Association of Diabetes Care & Education Specialists (ADCES). Accreditation represents a high level of quality and service to the community, and the ability to better meet the needs of Medicare recipients and others affected by diabetes in this community.

Now that the program has received accreditation, ACHC’s DSMES Program is now being offered to ACHC patients and to patients who receive an outside referral from other providers who have patients in this community that need diabetes education and need to learn how to self-manage their diabetes better.

The purpose of ACHC’s DSMES Program is to assist patients in managing their diabetes through diet, exercise, medication management, and problem solving. It will also allow people to learn more about how to reduce their risk, and to be monitored by Sue Warner, FNP-C and ACHC's Certified Diabetes Care and Education Specialist (CDCES), while they are in the program.

ACHC’s DSMES Program will be located at 205 N. Berkley Ave, in Council, Idaho, at the Federally Qualified Health Center (FQHC). ACHC will be offering individual diabetes education sessions with Sue Warner on Mondays, Tuesdays, and Wednesdays by appointment only. People wishing to participate in the program must receive a referral from a provider prior to scheduling an appointment.  

Diabetes education services utilize a collaborative process through which people with or at risk for diabetes work with a diabetes educator to receive individualized care to help them reach their health goals. Diabetes education is a covered Medicare benefit when delivered through an accredited program.

Free Student Physicals - for Jr. & Sr. High Athletes

Did you know that we offer FREE Sports Physicals year round at ACHC? Your child doesn’t even have to be a patient of ACHC to take advantage of this awesome opportunity! So, if your child plans to play sports this coming school year, beat the back-to-school rush, and call 208-253-4242 to schedule an appointment for a FREE sports physical at ACHC.
 
We recommend checking with your child’s school to see if they require a specific form for the sports physical. If a form is not brought to the appointment, ACHC will provide one from the Idaho High School Activities Association.

 

Prescription for Play

ACHC is offering Prescription for Play (P4P). Bricks and related educational materials are provided as prompts to engage caregivers around the benefits of play for the development of emotional, cognitive, creative, social and physical skills. This is all done while you're at a well-child check-up! P4P is only offered for children ages 18-36 months.

ACHC’s Powerful Tools for Caregivers (PTC) Course

PTC is a course designed to help family caregivers find balance while caring for a loved one. In this course, participants will learn tools to reduce stress, make tough decisions, navigate family dynamics, connect with fellow caregivers and build resources.  ACHC’s PTC courses are led by our fantastic CFO, Wendy Mott, and our amazing Chronic Care Manager, Susan Goudie-Adams. Each class offers unique tools to help navigate the journey of caregiving. This scripted, evidence-based workshop series is not a support group, however, creates a supportive network of fellow caregivers.

If you take care of a family member or their care recipient who is aged 60 or older, this in-person course is the class for you! YOU ARE NOT ALONE! GIVE IT A TRY-IT COULD CHANGE YOUR LIFE!

The date for the next PTC course has not been announced yet. Once a date has been determined, it will be announced in our clinic, on our social media pages, and right here on the website. When the date is announced a registration link will be provided. Registration for the course is for the entire 6-week series and will be required to participate.

WIC Services

Southwest District Health (SWDH) WIC Professionals are now coming to ACHC several times a year to do in-person WIC appointments. To learn when the next time it will be at ACHC, keep an eye on the event calendar on the home page of this site or go to the events page on the Adams County Health Center Facebook page.  You can also call the SWDH in Weiser at 208-549-2370 or the SWDH in Payette at 208-642-9321 to schedule your appointment or to ask any questions.

There are a limited number of in-person appointments available during each visit, and they will fill up extremely fast! If you are unable to get an in-person appointment, but need WIC services, SWDH WIC professionals also offer WIC appointments over the phone. To schedule, participants can call SWDH in at 208-549-2370, the SWDH in Payette at 208-642-9321 or SWDH in Caldwell at 208-455-5300 to schedule the appointment. Then a WIC staff member will call the participant at the scheduled time of the appointment. This is for all types of WIC appointments. 💙ðŸ¤ðŸ’™ðŸ¤

Mammogram screening is an important step in taking care of yourself and your health. Schedule your mammogram today, because you can’t take care of others if you don’t take care of yourself first. To schedule an appointment with the St. Luke's Mobile Mammogram, call 208-706-2055. To schedule an appointment with the Saint Alphonsus 3D Mobile Mammography Bus is at ACHC, call 208-367-8787.  We recommend verifying that St. Luke's or Saint Alphonsus is in network for your insurance prior to scheduling an appointment with either group of professionals.

DID YOU KNOW? If a person doesn’t have insurance or needs help paying for a mammogram, there is a Saint Alphonsus Financial Assistance form that can be filled out. If approved, it could cover the entire cost of the scan and the radiologist to read it. Additionally, if you qualified for financial assistance and needed additional scans for the radiologist, it would cover that as well. If you would like to know if you qualify for this Saint Alphonsus Financial Assistance before scheduling an appointment on their 3D Mobile Mammography bus, ACHC does have the form at our front desk. Just stop by and fill it out, and one of our wonderful ladies will fax the form to Saint Alphonsus for you. You will then be contacted by their team to find out if you qualify for the assistance.

 

Boise Mobile Veteran Center Vehicle

The Boise Mobile Vet Center Vehicle comes to ACHC several times during the year. You can also call The Boise Veteran's Center at 208-342-3612 and ask about when they will be at ACHC next. 

If you are a servicemember, veteran, or a family member of a servicemember or veteran, stop by and talk to United States Army Reserve Captain (Ret.) Daniel “Colt†Ashley, MSW, VOP, a member of the Veteran Outreach Program at the Boise Vet Center. Active Duty Servicemembers, Veterans and Family members are invited to walk-in to talk to him on that day. He can help you understand your VA benefits and services available, find resources locally & nationally, and discuss counseling options for those who are eligible.

The Boise Mobile Vet Center Vehicle is FREE, and they do not share information discussed with the military as the sessions are private and confidential. If you have questions about whether a particular service will be available that day, please call 208-342-3612.

 

Give Kids a Smile (GKAS)

The GKAS program, which generally begins each year in February, is a nationwide program that is ran by the American Dental Association (ADA) Foundation, and targets underserved children with FREE oral health care.

During a GKAS event at ACHC, dental professionals come together to provide FREE dental procedures such as dental examinations, cleanings, fillings, X-rays, extractions, and sealants for free to children aged 18 and under, who do not have dental insurance and need care. Other types of dental procedures may also be done during a GKAS event at ACHC if deemed necessary by the Dental Team. 

Check back as it gets closer to February 2026 to find out the date for our next GKAS event at ACHC, because once it is announced on social media and in our clinic, appointments fill up  fast! ACHC is proud to partner with Idaho State University, School of Dentistry as a rural training site for Dental Residency students.

Type 1 Diabetes Risk Screening

T1D FREE Screening Days at ACHC occur on the first Wednesday of every month! Just call 208-253-4242 to schedule an appointment! You don't have to have symptoms or a family history of T1D to be screened! This screening can be done on CHILDREN and ADULTS and is just a poke to the finger! It is available to anyone who would like to be screened for T1D, regardless of if you are a patient of ACHC. What is Type 1 Diabetes?

Did you know that over 90% of people diagnosed with type 1 diabetes don’t have a family connection? Type 1 diabetes is a chronic, autoimmune disease that can strike both children and adults. A simple screening test can help families determine, before symptoms even occur, if they may be at risk. Because most people do not have a family history of type 1 diabetes, symptoms and a diagnosis often come out of the blue. And because families don’t know what to look for, many people require hospitalization at diagnosis because their blood sugars are very high and the person is very sick. If not treated properly, people with T1D are vulnerable to health issues ranging from minor to severe. Most people with T1D spend the majority of their time with blood glucose levels outside the recommended healthy range, which can lead to potentially deadly episodes of hyperglycemia (high blood sugar) and hypoglycemia (low blood sugar). Chronic high blood sugar often causes devastating health complications later in life, including blindness, kidney failure, heart disease and nerve damage that can lead to amputations.

ACHC's Diabetes Prevention Program (DPP)

What is DPP?

DPP teaches that weight loss is the best way to decrease the risk of being diagnosed with type 2 diabetes. The program encourages members to work together as a team to support each other to reach weight loss goals and to stay motivated to take charge of their own health to prevent being diagnosed with type 2 diabetes. During the course, students will learn that weight loss can not only help them prevent becoming a type 2 diabetic, but that losing weight can also help lower blood pressure and cholesterol, which leads to an even healthier lifestyle.

ACHC’s Diabetes Self-Management Education and Support Program

Adams County Health Center’s (ACHC) Diabetes Self-Management Education and Support (DSMES) Program has achieved the significant milestone of accreditation by the Association of Diabetes Care & Education Specialists (ADCES). Accreditation represents a high level of quality and service to the community, and the ability to better meet the needs of Medicare recipients and others affected by diabetes in this community.

Now that the program has received accreditation, ACHC’s DSMES Program is now being offered to ACHC patients and to patients who receive an outside referral from other providers who have patients in this community that need diabetes education and need to learn how to self-manage their diabetes better.

The purpose of ACHC’s DSMES Program is to assist patients in managing their diabetes through diet, exercise, medication management, and problem solving. It will also allow people to learn more about how to reduce their risk, and to be monitored by Sue Warner, FNP-C and ACHC's Certified Diabetes Care and Education Specialist (CDCES), while they are in the program.

ACHC’s DSMES Program will be located at 205 N. Berkley Ave, in Council, Idaho, at the Federally Qualified Health Center (FQHC). ACHC will be offering individual diabetes education sessions with Sue Warner on Mondays, Tuesdays, and Wednesdays by appointment only. People wishing to participate in the program must receive a referral from a provider prior to scheduling an appointment.  

Diabetes education services utilize a collaborative process through which people with or at risk for diabetes work with a diabetes educator to receive individualized care to help them reach their health goals. Diabetes education is a covered Medicare benefit when delivered through an accredited program.
